The Hospice Team

When you elect hospice services, Pacific Crest Hospice will assign an interdisciplinary team to work with you and your family/caregivers so your care is coordinated to maximize your comfort and safety. The team will discuss your plan of care you so you confident and at ease with the treatment and care you will receive. Our team will keep your doctor informed about your progress.

Hospice Physician   Our hospice physician oversees each patient’s medical care and symptom management, working closely with the hospice team and the patient’s primary care physician to determine appropriate interventions.

Hospice Nurses  Our nurse case manager will make regularly scheduled visits; assessing comfort and providing symptom management. The nurse will educate patients and their families about disease progression, use of medication and daily care needs.

Certified Hospice Aides  The certified hospice aide provides personal care services under the supervision of the registered nurse. These duties may include assistance in activities of daily living (e.g., bathing, personal hygiene) exercise, and assistance in retaining self-help skills

Social Workers  Our social workers have specialized training in addressing the psychosocial, practical and financial needs of each patient. Services provided may include emotional support, counseling, education on and support in accessing community resources, and serving as a liaison with other agencies and providers. Social workers will also provide bereavement counseling.

Chaplain  Chaplains provide spiritual support and counseling to the patient and their family in a manner appropriate to the individual beliefs of each person. This may also include assisting with memorial services, serving as a friendly visitor, or connecting patients to spiritual support services within the individual faith tradition.

Volunteers  Direct Care Volunteers are an important part of the Pacific Crest Hospice team. Trained volunteers provide a wide range of loving support such as companionship, reading, playing or performing music, respite to the family, pet therapy, and facilitating special projects.

Additional support is available as necessary, including a consulting pharmacist and a dietitian.
